Licensed Vocational Nurse (Home Health)

This position offers skilled nursing care in a home health setting under the direct supervision of a Registered Nurse. As a Licensed Vocational Nurse, you will perform selected skilled nursing actions in the provision of curative, rehabilitative, palliative, or preventative care.

Responsibilities

• Assist the Registered Nurse in carrying out the plan of care and performing specialized procedures under physician's orders
• Recognize and understand the effects of social and economic problems upon patients and provide for their emotional and physical comfort and safety
• Educate patients and family members on appropriate self-care techniques
• Foster cooperative effort among the clinical care team by understanding the functions of other roles involved in patient care and actively participating in team and staff conferences
• Prepare clinical and progress notes, observe, record, and report patient symptoms, reactions, and changes in general physical and mental condition to the appropriate person

Required Qualifications

• Active California Licensed Vocational Nurse license
• One or more years of experience in a home health agency or hospital setting (home health experience preferred)
• Current CPR certification (online certification is not accepted)
• Valid driver's license without restrictions
• Reliable personal vehicle required
• Must successfully complete a criminal background check
