Licensed Practical/Vocational Nurse

Premier Home Health Providers Inc. is seeking a Licensed Practical/Vocational Nurse to provide direct patient care in the home setting. You will help patients manage their conditions, prevent rehospitalizations, and improve their health and quality of life during their recovery at home. This is a per diem position with a rate of $45 - $55 per visit.

Responsibilities

• Provide direct patient care in accordance with the state Nurse Practice Act
• Implement and follow individualized care plans established by the RN
• Educate patients and their families on self-care techniques and diagnosis-related information
• Document care accurately and in a timely manner
• Collaborate with the care team, including OT, PT, HHA, and other disciplines, and communicate patient needs to the RN or Clinical Supervisor
• Use supplies and equipment safely, adhering to aseptic technique
• Perform other duties as required for patient services

Required Qualifications

• Graduate of an accredited practical or vocational nursing program
• Active LPN or LVN license in California
• 3 or more years of nursing experience
• Current CPR certification with AHA
• Valid driver's license, reliable insured vehicle, and the ability to travel to patient homes
• Strong communication skills and a collaborative, patient-first mindset

Preferred Qualifications

• Home health or medical/surgical nursing background

Schedule and Compensation

• Per diem position with flexible scheduling
• Ability to convert to full-time based on performance if interested
