Education

  • Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN) in the state

Certifications

  • CPR certification

We are seeking a Licensed Practical/Vocational Nurse to provide direct patient care in our hospice program. Working under the supervision of a registered nurse, you will implement individualized plans of care, provide treatments, and collaborate with an interdisciplinary team to support positive patient outcomes for patients and their families.

Responsibilities

  • Provide direct patient care as defined by state Nurse Practice Act requirements
  • Implement the plan of care initiated by the registered nurse
  • Perform accurate and timely documentation consistent with the plan of care
  • Assess patient and family/caregiver needs and provide education pertinent to diagnosis and treatment plan
  • Assist the registered nurse or physician with specialized procedures, including preparing equipment, maintaining aseptic technique, and teaching patients appropriate self-care techniques
  • Coordinate home health services and report identified needs for other disciplines (HHA, OT, PT, MSW, ST, Dietician) to the registered nurse or Clinical Supervisor/Nursing Supervisor
  • Use equipment and supplies effectively and efficiently
  • Participate in personal and professional growth and development
  • Perform other duties as assigned by the registered nurse

Required Qualifications

  • Graduate of an accredited practical nurse or vocational nursing program
  • Three years of nursing experience, with at least one year of prior professional nursing experience in a community/home health or medical/surgical setting (effective January 1, 1998)
  • Current licensure as a Licensed Practical Nurse or Licensed Vocational Nurse in the state
  • Compliance with accepted professional standards and practice
  • Strong verbal and written communication and organizational skills
  • Current CPR certification
  • Valid driver's license with a personally owned, insured automobile in good working order that meets state and organizational requirements
