We are seeking a Licensed Practical/Vocational Nurse to provide direct patient care in our hospice program. Working under the supervision of a registered nurse, you will implement individualized plans of care, provide treatments, and collaborate with an interdisciplinary team to support positive patient outcomes for patients and their families.
Responsibilities
- Provide direct patient care as defined by state Nurse Practice Act requirements
- Implement the plan of care initiated by the registered nurse
- Perform accurate and timely documentation consistent with the plan of care
- Assess patient and family/caregiver needs and provide education pertinent to diagnosis and treatment plan
- Assist the registered nurse or physician with specialized procedures, including preparing equipment, maintaining aseptic technique, and teaching patients appropriate self-care techniques
- Coordinate home health services and report identified needs for other disciplines (HHA, OT, PT, MSW, ST, Dietician) to the registered nurse or Clinical Supervisor/Nursing Supervisor
- Use equipment and supplies effectively and efficiently
- Participate in personal and professional growth and development
- Perform other duties as assigned by the registered nurse
Required Qualifications
- Graduate of an accredited practical nurse or vocational nursing program
- Three years of nursing experience, with at least one year of prior professional nursing experience in a community/home health or medical/surgical setting (effective January 1, 1998)
- Current licensure as a Licensed Practical Nurse or Licensed Vocational Nurse in the state
- Compliance with accepted professional standards and practice
- Strong verbal and written communication and organizational skills
- Current CPR certification
- Valid driver's license with a personally owned, insured automobile in good working order that meets state and organizational requirements
