Licensed Practical/Vocational Nurse

Caring Hand Home Health Solutions is seeking a Licensed Practical/Vocational Nurse to provide direct patient care in the home setting. In this per diem role, you will support patients transitioning from medical facilities back to their homes, helping them heal, maintain safety, and achieve independence in their daily self-care activities. You will work as part of a multidisciplinary care team to deliver comprehensive, individualized care in a supportive environment.

Responsibilities

• Provide direct patient care in accordance with the state Nurse Practice Act
• Implement and follow individualized care plans established by the RN
• Educate patients and their families on self-care techniques and diagnosis-related information
• Document care accurately and in a timely manner
• Collaborate with the care team, including OT, PT, HHA, and other disciplines, and communicate patient needs to the RN or Clinical Supervisor
• Use supplies and equipment safely, adhering to aseptic technique
• Perform other duties as required for patient services

Required Qualifications

• Graduate of an accredited practical or vocational nursing program
• Active LPN or LVN license in California
• 3 or more years of nursing experience
• Current CPR certification with AHA
• Valid driver's license, reliable insured vehicle, and ability to travel to patient homes
• Strong communication skills and collaborative, patient-first mindset

Preferred Qualifications

• Home health or medical/surgical nursing background

Compensation and Schedule

• Per diem position with a rate of $45 to $55 per visit
• Flexible schedule
