Licensed Practical/Vocational Nurse

Affinity Hospice Care Inc. is seeking a Licensed Practical/Vocational Nurse to provide direct patient care in home hospice settings. In this per diem role, you will help patients and their families navigate end-of-life care with compassion and clinical expertise, working to keep patients safe and comfortable in their own homes.

Clinical Setting

This is a home-based hospice care position. You will travel to patient homes to deliver care and support.

Responsibilities

• Provide direct patient care in accordance with the state Nurse Practice Act
• Implement and follow individualized care plans established by the RN
• Educate patients and their families on self-care techniques and diagnosis-related information
• Document care accurately and in a timely manner
• Collaborate with the interdisciplinary care team, including occupational therapy, physical therapy, home health aides, and other disciplines
• Communicate patient needs to the RN or Clinical Supervisor
• Use supplies and equipment safely, adhering to aseptic technique
• Perform additional duties as required for patient services

Required Qualifications

• Graduate of an accredited practical or vocational nursing program
• Active LPN or LVN license in California
• Minimum 3 years of nursing experience
• Current CPR certification with the American Heart Association
• Valid driver's license and reliable insured vehicle
• Ability to travel to patient homes

Preferred Qualifications

• Home health or medical/surgical nursing background

Compensation and Schedule

This is a per diem position with a rate of $50 to $60 per visit. The role offers a flexible schedule with the potential to convert to full-time employment based on performance.

Travel Requirements

You must have a valid driver's license, a reliable insured vehicle, and the ability to travel to patient homes. Mileage is reimbursed for visits.
