Affinity Hospice Care Inc. is seeking a Licensed Practical/Vocational Nurse to provide direct patient care in home hospice settings. In this per diem role, you will support patients and their families during end-of-life transitions, helping patients remain safe and comfortable in their own homes while working alongside an interdisciplinary care team.

Clinical Setting

This is a per diem position providing home-based hospice care. You will travel to patient homes to deliver care and support.

Responsibilities

  • Provide direct patient care in accordance with the state Nurse Practice Act
  • Implement and follow individualized care plans established by the RN
  • Educate patients and their families on self-care techniques and diagnosis-related information
  • Document care accurately and in a timely manner
  • Collaborate with the interdisciplinary care team, including occupational therapy, physical therapy, home health aides, and other disciplines
  • Communicate patient needs to the RN or Clinical Supervisor
  • Use supplies and equipment safely, adhering to aseptic technique
  • Perform additional duties as required for patient services

Required Qualifications

  • Graduate of an accredited practical or vocational nursing program
  • Active LPN or LVN license in California
  • Minimum 3 years of nursing experience
  • Current CPR certification with American Heart Association (AHA)
  • Valid driver's license and reliable insured vehicle
  • Ability to travel to patient homes

Preferred Qualifications

  • Home health or medical/surgical nursing background

Compensation

  • Per diem rate of $50 to $60 per visit
  • Mileage reimbursement for patient-to-patient visits

Travel Requirements
