Affinity Hospice Care Inc. is seeking a Licensed Practical/Vocational Nurse to provide direct patient care in home hospice settings. In this per diem role, you will support patients and their families during end-of-life transitions, helping patients remain safe and comfortable in their own homes while working alongside an interdisciplinary care team.
Clinical Setting
This is a per diem position providing home-based hospice care. You will travel to patient homes to deliver care and support.
Responsibilities
- Provide direct patient care in accordance with the state Nurse Practice Act
- Implement and follow individualized care plans established by the RN
- Educate patients and their families on self-care techniques and diagnosis-related information
- Document care accurately and in a timely manner
- Collaborate with the interdisciplinary care team, including occupational therapy, physical therapy, home health aides, and other disciplines
- Communicate patient needs to the RN or Clinical Supervisor
- Use supplies and equipment safely, adhering to aseptic technique
- Perform additional duties as required for patient services
Required Qualifications
- Graduate of an accredited practical or vocational nursing program
- Active LPN or LVN license in California
- Minimum 3 years of nursing experience
- Current CPR certification with American Heart Association (AHA)
- Valid driver's license and reliable insured vehicle
- Ability to travel to patient homes
Preferred Qualifications
- Home health or medical/surgical nursing background
Compensation
- Per diem rate of $50 to $60 per visit
- Mileage reimbursement for patient-to-patient visits
