Licensed Practical Nurse/Medical Home Coordinator

Join TriHealth as a Licensed Practical Nurse/Medical Home Coordinator in a physician office setting where you will deliver compassionate care and support high-quality primary and specialty care. This part-time, day-shift position plays an essential role in creating positive patient experiences and collaborating with physicians and interdisciplinary healthcare teams.

Clinical Setting

You will work at 6350 Glenway Ave in an outpatient physician office environment, assisting physicians and registered nurses in providing clinical care and supervising care delivered by medical assistants.

Responsibilities

• Prepare and safely administer medications to patients in accordance with law, professional standards, LPN scope of practice, and organizational policy
• Interpret and execute physician orders accurately and document medications timely according to nursing standards
• Assist in answering medication questions and proactively educate patients and caregivers related to medications
• Perform technical nursing skills according to hospital and department policies, procedures, and accepted standards of practice
• Determine supply needs for the nursing department and order supplies necessary to maintain departmental operations
• Coordinate monthly turnover of Medication Administration Records (MARs) and participate in monthly MAR reviews
• Develop MARs for new patients as indicated
• Collect data that contributes to nursing assessment and document ongoing observations
• Communicate observations and findings to other healthcare team members
• Demonstrate knowledge and application of PACE regulations related to primary care provision
• Identify areas for self-improvement and maintain current knowledge in your area of practice
• Cooperate with clinical instructors to facilitate effective learning experiences for nursing students
• Demonstrate knowledge of organizational and unit changes

Required Qualifications

• Graduate of an approved technical, professional, or vocational program in nursing
• Current Licensed Practical Nurse (LPN) license
• Basic Life Support for Healthcare Providers (BLS) certification
• Ability to work within a multidisciplinary team
• 1-2 years of experience in a related field

Note: Current RN licensure is also acceptable.

This position requires consistent standing, walking, reaching, use of hands, thinking and reasoning, reading, talking, hearing conversation and other sounds, color vision, and visual acuity (both near and far). You will frequently engage in interpersonal communication and stooping. You will occasionally sit and kneel. You will consistently lift items under 50 lbs and over 50 lbs, push, and pull. Climbing is rarely required.
