EverTrue is seeking a Licensed Practical Nurse (LPN) for a full-time evening position at Brooking Park, a skilled nursing facility. In this role, you will provide direct nursing care to residents, coordinate individualized care plans with the interdisciplinary team, and ensure compliance with state and federal regulations and facility policies.
Responsibilities
- Practice nursing care within the scope of LPN licensure and in accordance with state Nurse Practice Act and facility policies
- Maintain current active LPN license and comply with all state and federal licensure rules and regulations
- Work collaboratively with the interdisciplinary team to develop and implement individualized plans of care for each resident
- Pass medications and oversee the medication ordering and administration process; administer medications and treatments as ordered
- Monitor administration of IV fluids, supplemental nourishments, and medications
- Initiate IV therapy if IV-certified
- Maintain working knowledge of observation, assessment, and documentation processes for the LPN level of practice, including care and service plans and MDS as applicable
- Utilize the electronic medical record to capture full clinical documentation in accordance with policy and procedures
- Observe resident status to assure quality care and coordinate with other resident services to ensure continuity of care
- Document medical information and maintain detailed, timely, and accurate reports
- Maintain a safe, comfortable, and functional environment by assessing potential hazards and implementing safety practices
- Oversee and direct certified staff and others as assigned to ensure facility standards of care and services
- Report and support investigative protocols related to staff or resident incidents and accidents in accordance with state and federal regulations
- Promote individual and team learning and provide teaching and role modeling of professional nursing practices to staff
Required Qualifications
- Active LPN license in state of practice
- Current CPR certification

Schedule and Shift
- Full-time position
- Evening shift
- Eligible for shift differentials: $2.00/hour for evenings, $1.50/hour for nights, $1.50/hour for weekends (rates apply in IL and MO; may vary in other states)
Physical Requirements and Working Conditions
- Ability to lift 30 pounds and push/pull 50 pounds
- Regularly required to stand, walk, and use hands to finger, handle, or feel objects, tools, or controls
- Frequently required to sit, reach with hands and arms, and stoop, kneel, crouch, or crawl
- Occasionally required to climb or balance
- Must have speech clarity, speech recognition, written expression, and sensory abilities
- Work environment includes a well-lighted and ventilated area with temperature control
- May be subject to emotionally upset residents or family members
- May be required to work beyond normal working hours and on weekends and holidays when necessary
- Potential exposure to fumes or airborne particles and toxic or caustic chemicals
- Potential exposure to infectious waste, diseases, and conditions including AIDS and Hepatitis B virus within guidelines of standard precautions
- Appropriate ergonomic and personal safety equipment is provided as needed
Compensation
- Pay grade 12: $23.55/hour to $37.75/hour
- Eligible for shift differentials
