UofL Health - Frazier Rehabilitation - Brownsboro is a 40-bed inpatient acute rehabilitation hospital dedicated to treatment and recovery for individuals with severe injuries or illnesses. We are seeking a Licensed Practical Nurse to provide direct patient care in our rehabilitation setting, supporting patients recovering from stroke, brain injury, neurological conditions, trauma, spinal cord injury, amputation, and orthopedic injury.

Clinical Setting

This position is located in Louisville, KY at our state-of-the-art rehabilitation facility. You will work full-time days providing care within a specialized rehabilitation program focused on maximizing patient health, function, and quality of life.

Responsibilities

  • Administer medications safely and accurately based on patient age and clinical condition
  • Follow proper medication administration procedures to minimize medication errors
  • Assist the RN with initial assessments and patient reassessments per policy
  • Identify and implement appropriate rehabilitation nursing interventions
  • Perform timely and accurate Quality Improvement assessments under RN supervision
  • Deliver care based on patient response to treatment and update the interdisciplinary plan accordingly
  • Assess and reassess pain; use appropriate pain management techniques and educate patients and families
  • Perform all patient care tasks in a manner that promotes patient safety and reduces medical errors
  • Develop and implement individualized teaching plans based on patient and family learning needs
  • Perform other duties as assigned

Required Qualifications

  • Active LPN license
  • CPR/BLS certification required
  • Basic Life Support certification required within 30 days of hire

Preferred Qualifications
