STGi is seeking a Licensed Practical Nurse/Licensed Vocational Nurse (LPN/LVN) to provide clinical and administrative nursing care at a Community Based Outpatient Clinic (CBOC). Working part-time under the direction of the clinic manager, PACT provider, and program manager, you will deliver a wide variety of nursing services to support primary care delivery.
Clinical Setting
You will work within a PACT (Primary Care Team) model at a VA community-based outpatient clinic, collaborating with providers, RN care managers, and support staff to deliver coordinated patient care.
Responsibilities
- Administer medications and demonstrate knowledge of pharmacology and medication administration techniques per facility protocols and state regulations
- Check in and check out patients as part of the PACT team workflow
- Participate in all staff meetings and PACT huddles
- Observe patients and report adverse reactions to medications, treatment, or changes in physical or mental condition
- Perform cardiopulmonary resuscitation (CPR) and assist during respiratory and cardiac arrest procedures
- Perform nursing procedures including dressing changes, suture removal, oxygen administration, wound irrigation, catheterization, injection administration, and venipunctures
- Prepare patients for laboratory procedures and examinations by providing information and instructions
- Set up and operate therapeutic and emergency equipment including cardiac monitors, EKG machines, oxygen delivery systems, automatic electronic defibrillators (AED), and portable suction
- Maintain universal precautions and infection control practices
- Document all pertinent patient information in medical records and VISTA (Veteran Health Information Systems and Technology Architecture) to ensure quality care delivery and continuity of care
- Maintain the VISTA clinic reminder tracking system for enrolled patients
- Complete VISTA Clinical Reminder Program documentation as outlined by site-specific VA Medical Center requirements
- Assist in scheduling patients for initial and follow-up appointments in accordance with VA access standards and medical necessity
- Inform patients regarding their plan of care, including lab and diagnostic test results
- Provide health educational materials and resources to patients and families
- Participate in the ongoing Performance Improvement Program between STGi, subcontractors, and the VA Medical Center
- Ensure all required reports are completed accurately
- Support clinic compliance with federal, state, local, JCAHO, OSHA, Veterans Affairs, and STGi safety and operational regulations
- Ensure patient information on access and wait time documentation is complete and accurate
- Maintain confidentiality of all patient information and support patients' privacy, rights, and safety
- Apply customer service techniques, cultural diversity awareness, and age-specific care approaches
- Document patient reminders, clinical interactions, and close encounters to ensure beneficial visits
- Make follow-up visits as needed
- Provide general clerical and administrative support coordination for the unit
- Perform other work-related duties as assigned
Required Qualifications
- LPN/LVN from an accredited nursing school
- Current, full, active, and unrestricted nursing license in New York
- Current Cardiopulmonary Resuscitation (CPR)/Basic Life Support (BLS) certification from American Heart Association or Military Training Network
- Knowledge of basic methods and procedures for coordinating and scheduling clinical care
- Thorough knowledge of variations in anatomy
- Knowledge and ability to apply practical nursing principles, procedures, and techniques
- Ability to assess patient condition, recognize adverse signs and symptoms, and react swiftly in emergency situations
- Ability to follow orders and work under close supervision while demonstrating initiative and independence
- Ability to provide individualized and group counseling, guidance, and health instruction focused on disease prevention and health maintenance
- Ability to provide educational material to patients and families regarding home health care activities
- Knowledge of pharmacological agents including desired effects, side effects, complications, and patient usage considerations
- Knowledge of age-specific customer service techniques with exceptional verbal, written, and telephone communication skills
- Ability to handle difficult situations with diplomacy and professionalism
- Knowledge of specialized terminology associated with the facility and in-depth knowledge of Primary Care Practice terminology
- Proficiency with Microsoft Office Software and Electronic Medical Records systems
- Strong computer skills and maneuverability
- Detail-oriented with ability to multitask
Preferred Qualifications
- One to three years of current clinical experience in a similar environment
- Outpatient clinical experience
- Hospital experience in Medical-Surgical, Intensive Care Unit, Emergency Room, or other hospital settings
Physical Requirements
- Moderate physical activity required
- Ability to handle average-weight objects up to 25 pounds
- Ability to stand and/or walk for more than four hours per day
- Required to respond to medical emergencies within the clinic, which may require fast walking or jogging to provide assistance
- Work is normally performed in a typical interior/office environment
Compensation
- Starting pay rate: $24.42/hour
