The opportunity

Job Description

UofL Health - Frazier Rehabilitation - Brownsboro is a 40-bed inpatient acute rehabilitation hospital dedicated to treating individuals recovering from severe injuries or illnesses. As a Licensed Practical Nurse (LPN), you will provide direct patient care under the supervision of a Registered Nurse, delivering treatments and supporting comprehensive rehabilitation programs for patients recovering from stroke, brain injury, neurological conditions, trauma, spinal cord injury, amputation, and orthopedic injury.

Clinical Setting

This position is located in Louisville, Kentucky at a state-of-the-art acute rehabilitation facility specializing in customized, individualized rehabilitation tailored to maximize patient health, function, and quality of life.

Schedule

Full-time nights, 7 p.m. to 7 a.m.

Responsibilities

  • Administer medications safely and accurately based on patient age and clinical condition
  • Follow proper medication administration procedures to minimize potential for medication errors
  • Assist the RN with initial assessments and patient reassessments per policy
  • Identify and implement appropriate rehabilitation nursing interventions
  • Perform timely and accurate Quality Improvement (QI) assessments under RN supervision
  • Deliver care based on patient response to treatment and update the interdisciplinary plan accordingly
  • Assess and reassess pain; use appropriate pain management techniques and educate patients and families
  • Perform all patient care tasks in a manner that promotes patient safety and reduces medical errors
  • Develop and implement individualized teaching plans based on patient and family learning needs
  • Perform other duties as assigned

Required Qualifications

  • Active LPN license
  • CPR/BLS certification
  • Basic Life Support certification required within 30 days of hire

Preferred Qualifications
