CoxHealth South is seeking a Licensed Practical Nurse or Graduate Practical Nurse for the Transitional Care Unit at our Springfield, Missouri location. This full-time night shift position (6:30 PM - 7:00 AM, 36 hours per week) focuses on providing direct care to adult and geriatric patients during their recovery from general medical conditions or post-operative procedures.
You will work as part of an interdisciplinary team dedicated to helping patients return home at their maximum level of functional ability. The unit serves patients across multiple specialties including cardiac, trauma, orthopedic, and neurological conditions.
Responsibilities
- Provide direct patient care including medication administration, dressing changes, and vital sign monitoring
- Assess patient condition and skin integrity
- Maintain accurate medical records and documentation
- Provide patients and families with support and education to facilitate successful transition home
- Demonstrate effective knowledge of scope of practice
- Manage time effectively in a multi-tasking environment
- Communicate clearly with patients, families, and the interdisciplinary team
- Apply strong critical thinking and problem-solving skills to patient care needs
Required Qualifications
- Graduate of an accredited practical nursing program
- For Licensed Practical Nurses: Active LPN license in Missouri and IV Certification
- For Graduate Practical Nurses: Must pass NCLEX-PN within 90 days of graduation
- BLS certification obtained within 90 days of hire
- Excellent verbal and written communication skills
- Strong critical thinking and problem-solving abilities
- Flexibility and ability to work in a multi-tasking environment
