Avamere Three Fountains is seeking a Licensed Practical Nurse for a full-time day shift position (6am-6pm) at our long-term care facility in Medford, Oregon. In this role, you will provide basic nursing care to residents under the supervision of a Registered Nurse, including vital signs monitoring, medication administration, wound care, and assistance with daily activities.
Responsibilities
- Measure and accurately record vital signs including temperature, pulse, blood pressure, and respirations
- Administer medications as prescribed by a physician, following facility protocols and under RN supervision
- Perform wound care including dressing changes and monitoring for signs of infection
- Assist residents with personal hygiene, dressing, bathing, and mobility
- Ensure residents receive proper nutrition and hydration, assisting with feeding as necessary
- Record patient history and current condition
- Observe and document changes in resident condition, reporting concerns to the RN
- Communicate effectively with residents, families, and other healthcare professionals
- Supervise certified nursing assistants in providing basic nursing care
- Collaborate with other healthcare professionals to ensure comprehensive resident care
- Maintain a safe environment for residents by following infection control and safety protocols
- Accurately document all care provided, including medications, vital signs, and observations
- Provide emotional support and comfort to residents and their families
Required Qualifications
- Nursing degree from an accredited college or university
- Current, unencumbered, active LPN license in Oregon
- Active CPR/BLS certification during employment

- Experience in long-term care, geriatric rehabilitation, sub-acute, or critical care clinical settings
- Experience with electronic medical records documentation
Schedule
- Full-time day shift, 6am-6pm
- Location: Avamere Three Fountains, 835 Crater Lake Ave, Medford, Oregon
