Licensed Practical Nurse

This Licensed Practical Nurse position provides direct medical care and support to residents living in a community home setting. You will work with individuals with developmental disabilities, mental health conditions, and other illnesses, focusing on routine health maintenance, behavioral understanding, and long-term advocacy in compliance with Agency for Persons with Disabilities (APD) standards.

Clinical Setting

You will work in a residential community home environment supporting vulnerable populations under state regulatory oversight.

Responsibilities

• Securely manage, order, and administer routine and psychotropic medications while carefully tracking adherence and potential side effects
• Review medication administration records (MARs) and medication changes for accuracy to prevent medication errors
• Observe residents for subtle physical or behavioral changes, particularly for non-verbal individuals who communicate distress through behavioral shifts
• Follow up on medical appointments, hospital discharges, and treatment recommendations
• Monitor and care for residents with specialized needs, including seizure protocols, G-tube feedings, and modified diet textures (mechanical soft or pureed)
• Partner with Direct Support Professionals (DSPs) to ensure individual care plans (ICPs) are properly followed during daily routines
• Maintain rigorous documentation adhering to state agency regulations regarding medical logs and incident reporting
• Accompany residents to medical appointments to accurately communicate health histories and advocate for their specialized care needs
• Participate in support plan meetings, following person-centered planning principles and ensuring each individual's choices and independence guide their care

Required Qualifications

• Active and unencumbered LPN license
• Current BLS/CPR certification

Preferred Qualifications

• Prior experience working with individuals with intellectual and developmental disabilities (IDD), autism, or in a long-term care setting
• High level of patience and strong non-verbal communication interpretation skills
• Crisis intervention and de-escalation training (such as CPI or MANDT)
• Strong working knowledge of medication management and basic clinical assessments

Compensation

• Salary: $58,000 annually

All potential employees must complete a background screening through the Florida Care Provider Background Screening Clearinghouse, as mandated by section 435.12, Florida Statutes. This screening is required for employment in positions providing services to vulnerable populations.
