Licensed Practical Nurse - SNF/Payson

Maple Dell Health and Rehab is seeking Licensed Practical Nurses to join our skilled nursing facility team in Payson. In this role, you will deliver direct care to residents, assess their needs, develop individual care plans, administer nursing care, and supervise Certified Nursing Assistants and other personnel in the delivery of nursing care.

We offer shift differentials for night and weekend shifts.

Responsibilities

• Assess resident needs and develop individual care plans
• Administer nursing care to assigned residents
• Supervise Certified Nursing Assistants and other personnel in nursing care delivery
• Work collaboratively with an interdisciplinary care team
• Make independent clinical decisions when circumstances warrant

Required Qualifications

• Current, unencumbered, active Licensed Practical Nurse (LPN) license in Utah
• Clinical assessment skills at the level necessary to meet job requirements
• Ability to make independent decisions when circumstances warrant
• Knowledge of nursing and medical practices and procedures, as well as laws, regulations, and guidelines pertaining to long-term care
• Effective interpersonal skills and ability to work with an interdisciplinary care team
• Basic understanding of computer technology, including email use
• Proven written and oral communication skills
• Proven decision-making and analytical skills
• Ability to relate positively, favorably, and cooperatively with employees, residents, family members, and personnel of outside agencies and organizations
• Positive attitude and desire to work with the aging population
