Licensed Practical Nurse - SNF/Payson (55071)

Maple Dell Health and Rehab is seeking Licensed Practical Nurses to join our skilled nursing facility team in Payson. In this role, you will deliver compassionate care to our residents while working as part of a dedicated interdisciplinary team.

Responsibilities

• Assess resident needs and develop individual care plans
• Administer nursing care to assigned residents
• Supervise Certified Nursing Assistants (CNAs) and other personnel in the delivery of nursing care
• Collaborate with the interdisciplinary care team to ensure high-quality outcomes

Required Qualifications

• Current, unencumbered, active Licensed Practical Nurse (LPN) license in Utah
• Demonstrate clinical assessment skills at the level necessary to meet job requirements
• Ability to make independent decisions when circumstances warrant such action
• Knowledge of nursing and medical practices and procedures, as well as laws, regulations, and guidelines that pertain to long-term care
• Effective interpersonal skills and ability to work with an interdisciplinary care team
• Basic understanding of computer technology, including email use
• Proven written and oral communication skills
• Proven decision-making and analytical skills
• Ability to relate positively, favorably, and cooperatively with employees, residents, family members, and personnel of outside agencies and organizations
• Positive attitude and desire to work with the aging population

Compensation

• Shift differentials provided for night shift and weekend shifts
