Serenity Assisted Living & Memory Care is seeking a Licensed Practical Nurse to support the health and safety of residents by providing nursing services and clinical oversight under the direction of the Clinical Nurse Supervisor. This role is ideal for an LPN looking to transition into a leadership-oriented clinical setting while continuing to make an impact in senior care.
You will work with older adults, including individuals living with dementia, in an assisted living and memory care environment. This position combines direct resident care, medication management, staff education, and clinical support.
Responsibilities
- Monitor resident health and safety through focused nursing observations and ongoing collection of health information
- Assist the Clinical Nurse Supervisor with resident assessments, reassessments, care plans, and service agreements as assigned
- Monitor residents for changes in condition and report concerns to the Clinical Nurse Supervisor
- Communicate changes in resident conditions and care needs to the Clinical Nurse Supervisor, physicians, staff, families, and other appropriate parties
- Assist with coordination of resident services with physicians, case managers, families, and other healthcare providers
- Maintain accurate and timely resident records, nursing notes, and required documentation
- Help ensure staff understand and appropriately implement individualized resident care plans and service needs
- Provide nursing guidance within the LPN scope of practice and participate in assigned on-call coverage
- Monitor medication management and administration practices to ensure compliance with physician orders, facility policies, and applicable regulations
- Coordinate medication orders, renewals, and necessary follow-up with physicians as assigned
- Monitor physician orders and assist with ensuring orders are signed and implemented within required timeframes
- Set up resident medications according to physician orders, individualized care plans, facility procedures, and applicable nursing standards
- Monitor medication administration practices and the appropriate use of medical equipment
- Identify and report medication errors, procedural concerns, and other medication-related issues
- Ensure proper medication storage, documentation, and disposal
- Assist with nursing-related orientation training for newly hired unlicensed personnel
- Provide education related to facility procedures, medication administration, and resident-specific care needs
- Assist with continuing education, in-service training, and refresher education to address regulatory requirements and staff development needs
- Support competency evaluations by observing staff performance, providing instruction, and communicating areas of concern
- Provide coaching, education, and reinforcement of appropriate care practices
- Assist with maintaining accurate documentation of staff training, education, and competency requirements
- Collaborate with the Clinical Nurse Supervisor to help ensure staff remain current with required training and competencies
- Provide performance feedback and contribute to annual evaluations of unlicensed personnel
- Assist the Clinical Nurse Supervisor in maintaining compliance with applicable federal, state, and local regulations
- Participate in licensing surveys, regulatory inspections, and corrective action processes as needed
- Assist in developing, reviewing, and implementing nursing policies, procedures, and required documentation
- Maintain current knowledge of assisted living regulations, nursing practices, and industry standards applicable to the LPN role
- Identify and report suspected resident abuse, neglect, or maltreatment in accordance with applicable reporting requirements and facility procedures
- Maintain resident privacy, confidentiality, dignity, and respect
- Identify safety concerns and participate in quality improvement initiatives
- Maintain effective communication and positive working relationships with residents, families, staff, and healthcare providers
- Collaborate with the Clinical Nurse Supervisor and leadership team to address resident care concerns and operational needs
- Participate in staff meetings, committees, and interdisciplinary discussions
- Respond appropriately to emergencies, safety hazards, fire drills, and other urgent situations
- Report injuries involving residents, employees, or visitors in accordance with facility procedures
- Provide assistance to other departments and participate in facility events as needed
- Keep the Clinical Nurse Supervisor informed of changes in resident condition, staff concerns, clinical issues, and other significant needs or problems
Required Qualifications
- Current, active, and unrestricted Licensed Practical Nurse (LPN) license in the State of Minnesota
- Compassion and sensitivity when working with older adults, including individuals living with dementia
- Strong nursing observation, critical thinking, and problem-solving skills appropriate to the LPN scope of practice
- Leadership abilities, including the ability to assist with training, coaching, and supporting unlicensed personnel
- Strong verbal and written communication skills
- Ability to work effectively with residents, families, staff, physicians, and other healthcare providers
- Strong organizational skills with the ability to prioritize responsibilities effectively
- Ability to maintain confidentiality and exercise sound professional judgment
- Flexibility and adaptability in responding to changing resident and facility needs
- Ability to complete and maintain required orientation, annual training, competency, and tuberculosis screening requirements
- Must successfully complete required background study requirements
- Valid driver's license if driving is required as part of assigned duties
Schedule
Monday through Friday, 8:00 AM to 4:00 PM. All applicants must be available to rotate on-call for one week every three weeks for medical questions and/or emergencies via phone or messages.
