David Raines Community Health Centers is seeking a Licensed Practical Nurse Scribe to provide basic and skilled nursing care to patients under the direction of an RN, NP, or Physician. In this role, you will work in a community health center setting, accompanying providers into patient rooms to document encounters in real-time, ensure accuracy of medical records, and assist with clinical tasks in a high-volume patient care environment.
Responsibilities
- Accurately document the provider's dictation, patient history, review of systems, past medical and surgical history, and physical exam findings
- Accompany the provider into patient rooms to document encounters in real-time
- Input diagnostic results, lab results, and medication lists as instructed by the provider
- Obtain vital signs, weight, and other measurements
- Instruct and assist patients in specimen collection as ordered
- Perform point of care testing such as urine pregnancy tests, urine dipsticks, hematocrits, and blood glucose testing
- Observe changes in patient condition and report to the provider when indicated
- Intervene in or assist with emergency situations
- Review health records prior to provider seeing patient and ensure lab and diagnostic test results are in medical records
- Prepare patients for examinations or procedures
- Prepare and administer oral, intramuscular, and subcutaneous medications according to provider orders and clinical policy
- Assist with clinical tasks such as preparing exam rooms and patient intake
- Maintain a clean, organized, and safe environment
- Report equipment malfunctions to the Nursing Supervisor or Clinic Manager
- Collect necessary equipment and supplies, checking equipment operation prior to procedures
- Provide patient education for diagnostic test preparation and special home health care
- Assist in ordering supplies while avoiding overstocking
- Properly dispose of contaminated sharps and trash
- Clean and check refrigerator temperatures as assigned
- Prepare exam and treatment rooms for the next day
- Maintain strict adherence to HIPAA and patient confidentiality regulations
- Maintain a quick, efficient, and accurate pace to support high-volume patient flow
Required Qualifications
- Graduate of an accredited practical nursing program
- Current Licensed Practical/Vocational Nurse license in the State of Louisiana
- Valid BLS certification from American Heart Association
- Ability to prioritize workload and multitask
- Attention to detail and critical thinking skills
- Proficiency in electronic medical record (EMR) systems
- Excellent verbal and communication skills
