Licensed Practical Nurse

This position provides direct nursing care to residents in a long-term care facility. As a Licensed Practical Nurse, you will work under the direction of the Director of Nursing Services and Nurse Supervisors to ensure the highest quality of care in accordance with federal, state, and local standards and regulations.

Responsibilities

• Provide direct nursing care to residents according to established care plans and facility policies
• Assist residents with daily hygiene care including bathing, oral care, eye care, ear care, facial care, hair care, and nail care
• Assist residents with bowel and bladder functions and collect specimens as required
• Keep residents dry by changing clothing and linen when soiled or wet
• Assist with lifting, turning, moving, positioning, and transporting residents into and out of beds, chairs, bathtubs, wheelchairs, and lifts
• Assist in transporting residents to and from appointments, activities, and social programs
• Administer professional nursing services including catheterization, tube feedings, suction, applying and changing dressings and bandages, colostomy and drainage bag care, and blood draws as required
• Perform lab tests, blood pressure monitoring, and other routine assessments per the resident care plan
• Admit, transfer, and discharge residents as required
• Assist in the care of dying residents and provide post-mortem care
• Report and document any changes observed in a resident's condition or behavior to the Charge Nurse
• Review care plans daily to ensure appropriate care is being rendered
• Complete medical forms, reports, evaluations, charting, and other administrative documentation
• Chart nurses' notes in an informative and descriptive manner reflecting the care provided and the resident's response
• Complete and file required recordkeeping forms and charts upon admission, transfer, and discharge
• Complete accident and incident reports as necessary and submit to the Director as required
• Report all discrepancies concerning physician's orders, diet changes, or charting errors to the Nurse Supervisor
• Receive and give nursing reports at the beginning and end of shift
• Report occupational exposures to blood, body fluids, infectious materials, and hazardous chemicals in accordance with facility policies
• Monitor assigned personnel to ensure they follow established safety regulations
• Recommend equipment and supply needs to the Nurse Supervisor
• Oversee resident needs, staff, and unit operations as needed
• Treat residents, family members, visitors, and team members with dignity and respect
• Maintain confidentiality of all resident and family information
• Report witnessed or reported allegations of resident abuse or neglect immediately to the Administrator, Director of Nursing, or Unit Manager
• Comply with facility personnel, safety, corporate, and nursing department policies and procedures
• Report to work on time and as scheduled
• Develop and maintain good working relationships with interdepartmental personnel

Required Qualifications

• Valid professional LPN license in good standing with the State
• Valid CPR certification
• Basic computer skills
• Ability to read, write, speak, and understand the English language
• Regular worksite attendance
• Ability to work beyond normal working hours and on weekends and holidays when necessary
• Ability to assist in the evacuation of residents during emergency situations
• Ability to perform essential position functions with or without reasonable accommodation

When assisting residents with lifting or carrying objects in excess of 25 pounds, you must use a transfer or gait belt or obtain the assistance of another employee.
