This PRN Licensed Practical Nurse position is based in an assisted living facility where you will provide direct nursing care to residents while upholding the organization's commitment to compassionate, dignified care. You will work collaboratively with the nursing and resident services teams to support resident health, safety, and well-being.
Clinical Setting
You will work in an assisted living facility serving residents who require varying levels of support with activities of daily living and health management.
Responsibilities
- Administer prescribed medications with 100% accuracy, including proper dosage verification and knowledge of medication interactions and side effects
- Maintain secure medication management practices, including controlled substance counts with off-going nursing staff and ensuring the medication cart remains locked at all times
- Assist with resident admissions, including obtaining pertinent information and completing all required admission forms and documentation
- Provide hands-on nursing care as needed to meet resident needs
- Complete and maintain required documentation including ECC and LNS logs, service plans, progress notes, and monthly summaries
- Follow all healthcare provider orders and ensure clear, accurate, and timely communication with nursing staff
- Supervise non-licensed nursing staff and participate in performance evaluations when requested
- Provide daily feedback to nursing associates on their performance
- Investigate incident reports in collaboration with the ADORS and recommend appropriate follow-up actions
- Respond promptly and courteously to resident requests for assistance, including calls from family and visitors
- Maintain a safe, clean, and orderly environment free of hazards and fall risks
- Ensure residents participate in scheduled activities as described in the activities calendar
- Accept calls from staff during nights and weekends
- Meet with oncoming and departing nursing staff to discuss resident conditions and shift occurrences
- Demonstrate excellent decision-making skills and manage priorities effectively
- Practice good body mechanics and safe working habits
- Maintain professional appearance and dress according to facility policy
- Comply with HIPAA regulations and all assisted living facility regulations
- Serve as essential personnel during emergencies and follow established emergency procedures
- Take initiative in seeking additional information and offering solutions
- Perform additional duties and special projects as assigned
Required Qualifications
- Valid state-specific nursing license (Licensed Practical Nurse)
- High school diploma or general education degree (GED)
- One to two years of related nursing experience or equivalent combination of education and experience
- Ability to read, analyze, and interpret professional journals, technical procedures, and governmental regulations
- Ability to write reports and business correspondence
- Ability to effectively present information and respond to questions from managers, clients, and the general public
- Ability to add, subtract, multiply, and divide using whole numbers, fractions, and decimals
- Ability to solve practical problems and interpret instructions in written, oral, diagram, or schedule form
- Knowledge of word processing software and electronic medical records
Physical Demands
- Regularly required to stand, walk, use hands, reach with hands and arms, and talk or hear
- Frequently required to stoop, kneel, crouch, or crawl
- Occasionally required to sit, climb, balance, taste, or smell
- Must occasionally lift and/or move up to 50 pounds
- Occasionally exposed to bloodborne pathogens
