Constellation Health Services is seeking a Licensed Practical Nurse (LPN) to provide compassionate end-of-life care and hospice services to patients in their homes and communities in the Bangor, ME area. This is a PRN position with approximately 16 hours per week. You will work under the guidance of an RN to implement care plans, manage symptoms, and support patients and families during this critical time.

Responsibilities

  • Deliver nursing care in accordance with the State Nurse Practice Act, with a focus on end-of-life care
  • Implement current nursing practices based on comprehensive assessments and care plans
  • Maintain accurate and timely documentation of patient services
  • Provide patient and family/caregiver education and support
  • Recognize and address changes in a patient's condition
  • Collaborate with interdisciplinary teams, including RNs, hospice aides, social workers, and other professionals
  • Participate in Hospice Interdisciplinary Team (IDT) meetings to ensure coordinated care
  • Use and maintain equipment and supplies effectively
  • Manage pain and symptoms, evaluating patient responses to treatments
  • Invest in personal and professional growth and contribute to quality improvement efforts

Required Qualifications

  • Graduate of an accredited practical nurse program
  • LPN license in the state of employment (Maine)
  • Understanding of hospice philosophy and the needs of the terminally ill
  • Basic knowledge of iOS operating systems

Preferred Qualifications

  • Experience in hospice, palliative care, community health home care, or medical/surgical settings
  • Background in clinical auditing and chart reviews
  • Wound care experience
