Licensed Practical Nurse (PRN, FT, PT)

Grandview Nursing & Rehabilitation in Danville, Pennsylvania is seeking a Licensed Practical Nurse for evening shift work in our skilled nursing facility. In this role, you will deliver high-quality patient care, work collaboratively with an interdisciplinary healthcare team, and support patients and their families throughout their recovery.

Responsibilities

• Provide direct patient care, including administering medications, wound care, and vital sign monitoring
• Develop and implement individualized care plans in collaboration with the healthcare team
• Assist patients with activities of daily living, including bathing, dressing, and grooming
• Maintain accurate and detailed records of patient care and treatment
• Collaborate with the interdisciplinary team to develop and implement discharge plans
• Provide education and support to patients and their families on disease management, self-care, and other health-related topics
