Licensed Practical Nurse

We are seeking a Licensed Practical Nurse to provide direct nursing care to residents in our community setting. You will work under the supervision of the Nurse Supervisor and RCC Services to ensure the highest quality of care in accordance with federal, state, and local standards and regulations.

Clinical Setting

This position serves an elderly and disabled resident population in a community-based long-term care environment. You will work with residents who may be ill, disabled, emotionally upset, or at times hostile, requiring compassionate and professional care.

Responsibilities

• Provide direct nursing care to residents in accordance with current federal, state, and local standards, guidelines, and regulations
• Assist residents with daily hygiene care including bathing, oral, ear, eye, facial, hair, and nail care
• Keep residents dry by changing clothing and linen when soiled or wet
• Assist residents with bowel and bladder functions and collect specimens as required
• Assist with lifting, turning, moving, positioning, and transporting residents into and out of beds, chairs, bathtubs, wheelchairs, and lifts
• Administer professional nursing services including catheterization, tube feedings, suction, applying and changing dressings and bandages, colostomy and drainage bag care, and blood draws
• Perform lab tests, blood pressure monitoring, and other routine assessments as required per the resident care plan
• Admit, transfer, and discharge residents as required
• Report and document any changes observed in a resident's condition or behavior to the Charge Nurse
• Assist in the care of dying residents and provide post-mortem care
• Assist in the admission and discharge of residents
• Assist with transporting residents to and from appointments, activities, and social programs
• Complete and maintain required medical forms, reports, evaluations, charting, and recordkeeping forms upon admission, transfer, and discharge
• Chart nurses' notes in an informative and descriptive manner that reflects the care provided and the resident's response to care
• Complete accident and incident reports as necessary and submit to the Director
• Report all discrepancies concerning physician's orders, diet changes, or charting errors to the Nurse Supervisor
• Review complaints and grievances filed by assigned personnel and make appropriate reports to the Nurse Supervisor
• Receive and give nursing reports at the beginning and end of shift
• Report occupational exposures to blood, body fluids, infectious materials, and hazardous chemicals in accordance with community policies
• Monitor assigned personnel to ensure they follow established safety regulations
• Recommend equipment and supply needs to the Nurse Supervisor
• Review care plans daily to ensure appropriate care is being rendered
• Oversee resident needs, staff, and unit operations as needed
• Maintain confidentiality of all resident and family information
• Treat residents, family members, visitors, and team members with dignity and respect
• Report witnessed or reported allegations of resident abuse or neglect immediately to the Executive Director, RCC, or Unit Manager
• Comply with the community's personnel, safety, and corporate policies and procedures
• Comply with the nursing department's policies and procedures
• Report to work on time and as scheduled

Required Qualifications

• Valid professional LPN license in good standing with the State
• Basic computer skills
• Ability to read, write, speak, and understand the English language
• Regular worksite attendance
• Ability to work beyond normal working hours and on weekends and holidays when necessary
• Ability to assist in the evacuation of residents during emergency situations
• Ability to perform the essential functions of the job with or without reasonable accommodation
