Licensed Practical Nurse - PDN / Home Health $28 -

Maxim Healthcare is hiring a Licensed Practical Nurse (LPN) to provide direct patient care in home health and private duty nursing settings. You will work with pediatric and/or adult patients in their own homes, delivering skilled nursing care, medication administration, and patient education.

Responsibilities

• Utilize the nursing process to plan, implement, and evaluate patient care and services
• Initiate the plan of care when directed and request necessary revisions from the prescriber as patient status and needs change
• Incorporate therapeutic, preventive, and rehabilitative nursing actions based on patient diagnosis and established goals
• Intervene in response to changes in patient condition and report changes to the appropriate physician or supervisor
• Administer medications and treatments as prescribed, and monitor and document the effectiveness of interventions
• Review plan of care and program protocols to identify areas for healthcare teaching, care coordination, goal setting, and discharge planning

Schedule

• Day shift and night shift openings available
• 8-, 10-, and 12-hour shifts available

Required Qualifications

• Current LPN/LVN license in the state where you will practice
• Current TB test or chest X-ray
• Current BLS certification

Preferred Qualifications

• One year of prior professional nursing experience
