LICENSED PRACTICAL NURSE Part-Time 7a - 3:30pm Every other WKND

Parc Communities, a senior living provider based in Atlanta, is seeking a Licensed Practical Nurse to provide assisted living, memory care, and personal care services to residents. This part-time position works every other weekend from 7:00 AM to 3:30 PM and reports to the Resident Care Director.

Clinical Setting

You will work in an upscale senior living community providing care to residents in assisted living, memory care, and personal care home settings. The role requires collaboration with physicians, management, and interdisciplinary care teams to ensure quality and safe delivery of health services.

Responsibilities

• Perform physical assessments of residents and identify problems related to physiological, psychological, environmental, and social factors
• Administer medications per physician orders and observe for adverse reactions
• Meet regularly with the Resident Care Director to communicate care plans, challenges, and required support
• Collect and analyze pertinent health-related information
• Obtain physician orders prior to initiating or changing treatments
• Assess resident life history and preferences; maintain communication documentation
• Observe and report changes in resident functional status to the Resident Care Director and care staff
• Implement plans of care formulated by the physician and Resident Care Director
• Utilize safe and appropriate nursing techniques and skills in providing resident care
• Report pertinent changes in patient condition to the Resident Care Director and/or physician
• Report changes in patient status such as hospitalizations, discharges, and physician appointments
• Provide accurate and timely documentation consistent with the plan of care
• Assess and provide resident and family education pertinent to diagnosis and plan of care
• Communicate physician feedback with patients and/or families
• Follow established guidelines concerning drug storage
• Follow infection control and universal precautions when providing patient care
• Communicate directly with other disciplines involved in resident care, including weekends
• Participate in team meetings focused on resident problems
• Maintain a safe and healthy work environment using safe work methods and procedures
• Assist residents in learning appropriate self-care activities
• Organize and monitor resources necessary for meeting resident needs
• Recognize emergencies and life-threatening situations and initiate appropriate actions
• Document all information in a timely and accurate manner
• Maintain confidentiality and resident rights regarding all personal information
• Contact the office with any changes in visit schedules
• Participate in staff development activities and assist with orientation
• Follow dress code demonstrating appropriate appearance and personal hygiene

Required Qualifications

• Graduation from an accredited Licensed Vocational Nurse (LVN) or Licensed Practical Nurse (LPN) program
• Current LVN/LPN license in the state
• Current CPR certification
• Two to four years of nursing experience in a healthcare facility
• Experience in geriatric nursing, emergency care, and first aid
• Ability to obtain valid continuing education credits as required by the state
• Ability to cope with the mental and emotional stress of the position
• Ability to work effectively with physicians, management, employees, and outside contacts
• Ability to make independent decisions when circumstances warrant
• Proficiency with computer systems for electronic medical records and electronic medication administration records (EMAR)
• Strong attention to detail
• Ability to work as a team member

Preferred Qualifications

• Training in the special needs of cognitively impaired residents, including Alzheimer's and dementia care

Physical Demands and Work Environment

• Regularly required to stand, bend, talk, and hear
• Frequently required to walk
• Frequently required to use hands and fingers to touch, handle, feel, and reach with hands and arms
• Ability to lift, push, or pull up to 50 pounds using proper body mechanics
• Ability to sit, stand, and walk for extended periods
• Ability to climb flights of stairs
• Ability to assist residents to roll in bed, sit up from lying position, stand from sitting position, transfer to and from wheelchair, and provide stand-by assistance during ambulation or activities of daily living
• Possible exposure to unpleasant odors
• Possible exposure to chemicals as identified in the MSDS Manual
• Possible exposure to verbal aggression from vendors, guests, residents, and/or family members
• Continuous exposure to clients and family members who may be under stress
• May be subjected to hostile and emotionally upset residents, family members, personnel, and visitors

• Every other weekend
