We are seeking a Licensed Practical Nurse to deliver nursing care to residents in an assisted living setting in New York. You will work within the scope of an LPN license, managing a range of clinical responsibilities including catheter care, wound care, dressing changes, nebulizer treatments, ostomy care, and medical equipment management. You will also manage medication administration, supervise resident care staff, maintain resident records, and coordinate with families and healthcare providers.
Clinical Setting
This position is based in an assisted living or long-term care residence in New York, where you will provide direct nursing care to residents and supervise Resident Care Assistants (RCAs) and Home Health Aides (HHAs).
Responsibilities
- Coordinate and deliver nursing care within the scope of an LPN, including catheter care, wound care, dressing changes, nebulizer treatments, ostomy care, and medical equipment management
- Administer oral medications, injections, and PRN medications; assist residents with self-administration of medication
- Supervise care provided by HHAs and RCAs to residents
- Maintain positive collaborative relations with residents, families, and provider partners
- Report significant incidents and changes in resident condition to Nursing Leadership, Primary Care Providers, and families as indicated
- Review and become familiar with each assigned resident's Individualized Service Plan
- Assist in training and orientation of resident care staff
- Document care and services in the electronic health record using nursing best practices and documentation standards
- Follow all facility policies and procedures
- Attend and participate in scheduled in-service programs, training programs, and staff meetings
- Assist with scheduling and staffing of resident care staff as needed
- Provide coaching and mentoring to resident care team members
- Follow emergency procedures and comply with facility safety practices
- Participate in health, wellness, and lifestyle programs and assist with all aspects of resident care and experience as needed
Required Qualifications
- Active Licensed Practical Nurse license in the state of New York
- Basic first aid certification
- CPR certification
- Ability to convey and understand information and ideas in English
- Ability to handle workplace problems effectively, including anticipating, preventing, identifying, and solving problems
- Ability to assimilate complex information from multiple sources and adjust or modify to meet situational constraints
- Ability to prioritize departmental functions to meet due dates and deadlines
- Ability to stand for long hours and walk long distances without fatigue
- Ability to utilize technology for communication and operational compliance
Preferred Qualifications
- Experience in assisted living, post-acute, or long-term care settings

Friday and Saturday, 7:00 AM to 3:00 PM or 3:00 PM to 11:00 PM
