Licensed Practical Nurse or Registered Nurse

DMC Primary Care seeks an experienced Licensed Practical Nurse (LPN) or Registered Nurse (RN) to join our primary care team in Windham, NH. This full-time position offers the opportunity to care for patients with a wide variety of medical conditions, from acute care issues to wellness visits, while building long-term patient relationships in a fast-paced, supportive team environment.

Clinical Setting

You will work in our Windham, NH clinic, which is part of DMC Primary Care, a physician-owned independent practice providing comprehensive family care since 1964. The practice operates multiple offices throughout New Hampshire.

Schedule

This is a full-time position requiring physical presence in the Windham office 4 days per week for 36 hours. The schedule includes rotating Saturdays every 12 weeks at our Londonderry clinic.

Responsibilities

• Perform nurse visits including INR checks, coumadin management, vaccine administration, medication administration, heart monitor placement, EKG, sleep device instruction, and pulmonary function testing
• Manage triage calls from patients, families, care management teams, and other entities, applying nursing judgment to questions regarding medications, prescriptions, referral requests, immunization needs, lab results, and illness symptoms
• Manage complex multidisciplinary messages from providers and notify patients of abnormal laboratory and imaging results requiring further work-up
• Serve as liaison between providers, care management teams, and patients regarding quality-of-care initiatives through team huddles, face-to-face communication, and telephone contact on a daily basis
• Participate in health education pertinent to preventive care as needed

Required Qualifications

• Graduation from an accredited school of nursing
• Current State of New Hampshire Nurse or LPN license
• Minimum of two years of acute care or ambulatory care experience
• Ability to communicate effectively, both orally and in writing
