DMC Primary Care seeks an experienced Licensed Practical Nurse (LPN) or Registered Nurse (RN) to join our primary care team in Windham, NH. This full-time position offers the opportunity to care for patients with a wide variety of medical conditions, from acute care issues to wellness visits, while building long-term patient relationships in a fast-paced, supportive team environment.
Clinical Setting
You will work in our Windham, NH clinic, which is part of DMC Primary Care, a physician-owned independent practice providing comprehensive family care since 1964. The practice operates multiple offices throughout New Hampshire.
Schedule
This is a full-time position requiring physical presence in the Windham office 4 days per week for 36 hours. The schedule includes rotating Saturdays every 12 weeks at our Londonderry clinic.
Responsibilities
- Perform nurse visits including INR checks, coumadin management, vaccine administration, medication administration, heart monitor placement, EKG, sleep device instruction, and pulmonary function testing
- Manage triage calls from patients, families, care management teams, and other entities, applying nursing judgment to questions regarding medications, prescriptions, referral requests, immunization needs, lab results, and illness symptoms
- Manage complex multidisciplinary messages from providers and notify patients of abnormal laboratory and imaging results requiring further work-up
- Serve as liaison between providers, care management teams, and patients regarding quality-of-care initiatives through team huddles, face-to-face communication, and telephone contact on a daily basis
- Participate in health education pertinent to preventive care as needed
Required Qualifications
- Graduation from an accredited school of nursing
- Current State of New Hampshire Nurse or LPN license
- Minimum of two years of acute care or ambulatory care experience
- Ability to communicate effectively, both orally and in writing
