The opportunity

Job Description

Oak Hill School is seeking a Licensed Practical Nurse (LPN) to support the health, safety, and educational success of students with intellectual and developmental disabilities in our Special Education School Program. As a Classroom LPN, you will provide direct nursing care, health monitoring, and treatment implementation while collaborating with educational staff to help students participate safely in school activities.

Clinical Setting

You will work in a special education school environment serving students with complex medical and developmental needs. This role combines clinical nursing expertise with educational support, requiring you to work as part of an interdisciplinary team including special education teachers, therapists, and support personnel.

Responsibilities

Nursing Care and Health Services

  • Provide direct nursing care and treatments necessary to support student health and educational participation
  • Administer medications and treatments in accordance with physician orders, nursing standards, and school policies
  • Monitor student health status and respond appropriately to illness, injury, and emergencies
  • Implement and maintain individualized healthcare plans
  • Provide care for students with complex medical needs, including tracheostomy (trach) and gastrostomy tube (G-tube) management
  • Maintain accurate and timely health records and documentation

Student Support and Educational Collaboration

  • Work collaboratively with teachers, therapists, and support personnel to integrate health needs into daily programming
  • Assist in supporting student engagement, participation, and success within the classroom environment
  • Promote a positive, inclusive, and supportive school climate that encourages learning and personal growth
  • Support students' social, emotional, and behavioral development as part of the interdisciplinary team

Communication and Coordination

  • Communicate effectively with the Nurse Supervisor, educational staff, families, and external healthcare providers as appropriate
  • Participate in team meetings and contribute to collaborative student planning
  • Ensure compliance with all applicable nursing regulations, school policies, and Oak Hill procedures

Professional Development

  • Engage in ongoing professional learning and training opportunities
  • Demonstrate professionalism, collaboration, leadership, and a commitment to continuous improvement

Schedule

40 hours per week, Monday through Friday, 7:30 AM to 3:30 PM. Full-time, union position, non-exempt.

Required Qualifications

  • Active Connecticut Licensed Practical Nurse (LPN) license
  • Minimum of three years of licensed nursing experience
  • Experience working with pediatric populations
  • Experience providing care to individuals with tracheostomies (trachs) and gastrostomy tubes (G-tubes)
  • Valid driver's license
  • At least 18 years of age
  • Strong clinical assessment, documentation, communication, and organizational skills
  • Ability to work effectively within an interdisciplinary educational and healthcare environment
