Job Description
Mohawk Meadows Rehab and Nursing Center is seeking a Licensed Practical Nurse to provide high-quality, person-centered care to residents in our facility. In this role, you will work with an interdisciplinary team to support the physical, emotional, and psychological well-being of our residents.
Responsibilities
- Assess residents' physical, emotional, and psychological needs
- Develop and implement individualized care plans
- Administer medications and treatments as prescribed by physicians
- Monitor residents' conditions and report changes to healthcare providers
- Provide direct care to residents, including personal care, wound care, and assistance with activities of daily living
- Monitor and maintain accurate records of residents' care
- Collaborate with interdisciplinary teams to ensure comprehensive care
- Participate in quality improvement initiatives to improve patient outcomes
- Develop and maintain relationships with residents, families, and interdisciplinary teams
