Licensed Practical Nurse

Mohawk Meadows Rehab and Nursing Center is seeking a Licensed Practical Nurse to provide high-quality, person-centered care to residents in our facility. In this role, you will work with an interdisciplinary team to support the physical, emotional, and psychological well-being of our residents.

Responsibilities

• Assess residents' physical, emotional, and psychological needs
• Develop and implement individualized care plans
• Administer medications and treatments as prescribed by physicians
• Monitor residents' conditions and report changes to healthcare providers
• Provide direct care to residents, including personal care, wound care, and assistance with activities of daily living
• Monitor and maintain accurate records of residents' care
• Collaborate with interdisciplinary teams to ensure comprehensive care
• Participate in quality improvement initiatives to improve patient outcomes
• Develop and maintain relationships with residents, families, and interdisciplinary teams
