Licensed Practical Nurse (LPN)/Registered Nurse (RN)

Maple Health and Rehab is seeking a Licensed Practical Nurse (LPN) or Registered Nurse (RN) to join our long-term care facility. This full-time, weekday position offers the opportunity to provide personalized care to residents in a supportive environment that includes a dedicated memory care unit and a Rehab to Home short-term unit. We maintain above-average staff-to-resident ratios to ensure quality care and meaningful resident relationships.

Clinical Setting

Our facility specializes in long-term care with dedicated memory care services and short-term rehabilitation programs. We prioritize personalized, compassionate care and foster a collaborative healthcare team environment.

Responsibilities

• Consult and coordinate with healthcare team members to assess, plan, implement, and evaluate patient care plans
• Prepare and administer medications orally, subcutaneously, and intravenously; record administration and report adverse reactions according to facility policy
• Record patients' medical information and vital signs
• Monitor and adjust specialized equipment used on patients; interpret and record electronic displays
• Initiate corrective action when patients display adverse symptoms
• Provide bedside care for a wide variety of medical patients
• Change dressings, insert catheters, and start intravenous lines
• Prepare equipment and assist physicians during patient examinations and treatments
• Respond to life-saving situations based on nursing standards and protocol
• Participate in discharge planning
• Utilize effective time management skills
• Record all care information concisely, accurately, and completely in a timely manner using appropriate formats and documentation systems
• Perform other position-related duties as assigned

Required Qualifications

• Current RN or LPN license in the state

Preferred Qualifications

• At least one year of LPN or RN experience in a long-term care setting

Schedule

• Full-time, weekdays
