Jefferson Rehabilitation is seeking Licensed Practical Nurses (LPNs) and Registered Nurses (RNs) to provide direct, individualized nursing care in our long-term care community. This role offers the opportunity to work in a smaller facility setting that prioritizes personalized resident care, manageable workloads, and a supportive team environment.

Clinical Setting

Responsibilities

  • Provide direct and individualized nursing care based on scientific nursing principles
  • Consult and coordinate with healthcare team members to assess, plan, implement, and evaluate patient care plans
  • Prepare and administer medications orally, subcutaneously, and intravenously; record administration and report adverse reactions
  • Record patient medical information and vital signs
  • Monitor and adjust specialized equipment; interpret and record electronic displays
  • Initiate corrective action when patients display adverse symptoms
  • Provide bedside care for a wide variety of medical patients
  • Change dressings, insert catheters, and start IVs
  • Prepare equipment and assist physicians during patient examination and treatment
  • Respond to life-saving situations based on nursing standards and protocol
  • Participate in discharge planning
  • Record all care information concisely, accurately, and completely in a timely manner
  • Utilize effective time management skills
  • Perform other position-related duties as assigned

Required Qualifications

  • Current LPN or RN license in the state
  • Up-to-date First Aid and CPR certification

Preferred Qualifications

  • At least one year of experience as an LPN or RN in a long-term care setting
  • Knowledge of Matrixcare
