Licensed Practical Nurse (LPN)/Registered Nurse

This position provides direct nursing care to residents in a long-term care and assisted living facility, with responsibility for supervising day-to-day nursing activities in accordance with federal, state, and local regulations.

Responsibilities

• Observe residents for changes in condition and report to physicians and healthcare professionals
• Supervise and direct nursing assistants in accordance with facility regulations and guidelines
• Meet with assigned nursing staff and support personnel to plan shift services, programs, and activities
• Admit, transfer, and discharge residents as required
• Complete accident and incident reports
• Chart nurses' notes in an informative and descriptive manner reflecting care provided and resident response
• Perform routine charting duties in accordance with established documentation policies
• Order prescribed medications and recommend supplies and equipment as necessary
• Review medication administration records for completeness and accuracy of physician orders
• Review resident care plans for appropriate goals, problems, approaches, and revisions based on nursing needs
• Ensure nurses' notes reflect that care plans are being followed
• Ensure all nursing personnel are knowledgeable of resident rights and responsibilities
• Review resident complaints and grievances and report actions taken to resolve them to the Executive Director
• Interpret department policies and procedures to personnel, residents, visitors, and government agencies

Required Qualifications

• Current, unencumbered, active license to practice as an RN or LPN in Illinois
• Ability to make independent decisions when circumstances warrant
• Ability to relate information concerning a resident's condition
• Ability to deal tactfully with personnel, residents, family members, visitors, government agencies, and the public
• Knowledge of nursing and medical practices and procedures
• Knowledge of laws, regulations, and guidelines governing dietary functions in long-term care facilities
• Leadership ability and willingness to work harmoniously with and supervise other personnel
• Ability to maintain a positive professional manner and manage time efficiently
• Ability to promote a team approach and participate in problem-solving methods
• Commitment to providing care with respect and dignity for residents
• Ability to report all complaints, allegations of abuse, misappropriation of funds, and compliance issues immediately
• Ability to maintain confidentiality and discretion with sensitive information
• Enjoyment of working with the senior population
• Ability to perform job duties in alignment with the facility's mission of providing gracious senior living in a Christian environment

Physical and Mental Requirements

• Ability to move intermittently throughout the work day
• Ability to cope with the mental and emotional stress of the position
• Ability to push, pull, move, and/or lift a minimum of 75 pounds to a minimum height of 3 feet
• Ability to push, pull, move, and/or carry 75 pounds a minimum distance of 20 feet
• May be necessary to assist in the evacuation of residents during emergency situations
• Must meet general health requirements set forth by facility policies
