Licensed Practical Nurse LPN

We are seeking Licensed Practical Nurses (LPNs) to join our care team on day or night shift positions (7 AM to 7 PM or 7 PM to 7 AM). This role offers the opportunity to provide direct patient care in a long-term care setting, working with residents who require skilled nursing support. We are looking for individuals with genuine empathy and patience who are committed to delivering compassionate care.

Responsibilities

• Conduct patient assessments, follow established protocols, and report any medical or behavioral changes
• Document all findings and maintain accurate medical records
• Perform admission duties including completing assessments, medication administration records (MAR), treatment administration records (TAR), plan of service (POS), reviewing chart setup, and processing physician orders
• Manage patient discharges by completing required documentation, notifying physicians and guardians, and arranging transportation
• Provide wound care including irrigation, packing, and dressing removal according to facility protocols
• Lead and supervise Certified Nursing Assistants (CNAs) and Certified Medical Technicians (CMTs), ensuring compliance with facility protocols and policies
• Respond effectively to emergency situations

Required Qualifications

• Active LPN license

• Day shift (7 AM to 7 PM) or night shift (7 PM to 7 AM)
