Licensed Practical Nurse LPN

North Capitol Nursing and Rehabilitation is seeking a Licensed Practical Nurse (LPN) to provide direct care to residents in a long-term care setting. In this role, you will develop meaningful relationships with residents while utilizing nursing skills to promote their physical and psychosocial well-being. You will work collaboratively with nursing and facility teams to deliver coordinated, comprehensive care.

Schedule

• Full-time days and evenings
• Part-time nights

Responsibilities

• Provide direct nursing care to residents
• Manage medications with proficiency and accuracy
• Conduct thorough assessments and accurately document changes in resident condition
• Collaborate with nursing and other facility teams to ensure coordinated care
• Promote teamwork within the care team to meet resident needs
• Support a respectful and positive work environment through effective communication
• Demonstrate C.A.R.E. values (Compassion, Accountability, Relationships, Excellence) with residents, families, and staff

Required Qualifications

• Current and valid Licensed Practical Nurse license in the state of Indiana
• Proficient medication management skills
• Ability to conduct thorough assessments and accurately document changes in resident condition
• Excellent communication and interpersonal skills
• Strong passion for geriatric nursing and commitment to senior care excellence
