Community Nursing and Rehabilitation is seeking a Licensed Practical Nurse (LPN) to provide direct care to residents in a long-term care setting. In this role, you will develop meaningful relationships with residents while utilizing nursing skills to promote their physical and psychosocial well-being. You will work collaboratively with nursing and facility teams to deliver coordinated, comprehensive care that exceeds resident needs.

This position is based in a community nursing and rehabilitation facility serving seniors. You will work with a care team dedicated to geriatric nursing and senior care excellence.
Schedule
- Full-time evenings and nights
- Part-time nights
Responsibilities
- Provide direct nursing care to residents
- Manage medications with proficiency and accuracy
- Conduct thorough assessments and accurately document changes in resident condition
- Collaborate with nursing and facility teams to ensure coordinated care
- Promote teamwork within the care team
- Create a supportive and engaging atmosphere for residents
- Support a respectful and positive work environment through effective communication
- Demonstrate Compassion, Accountability, Relationships, and Excellence (C.A.R.E.) values in all interactions with residents, families, and staff
Required Qualifications
- Current and valid Licensed Practical Nurse license in the state of Indiana
- Proficient medication management skills
- Ability to conduct thorough assessments and accurately document changes in resident condition
- Strong passion for geriatric nursing and commitment to senior care excellence
- Excellent communication and interpersonal skills
