Licensed Practical Nurse LPN

Heritage Park is seeking a Licensed Practical Nurse (LPN) to join our senior care community. In this role, you will develop meaningful relationships with residents by learning their life stories while utilizing your nursing skills to promote their physical and psychosocial well-being. You will work as part of a collaborative care team to deliver excellent resident-centered care in a supportive environment.

Schedule

• Full-time day and evening shifts available
• Monday through Friday

Responsibilities

• Provide direct nursing care to promote the physical and psychosocial well-being of residents
• Manage medications with proficiency and accuracy
• Conduct thorough assessments and accurately document changes in resident condition
• Collaborate with nursing and other facility teams to ensure coordinated and comprehensive care
• Promote teamwork within the care team to exceed resident needs
• Create a comforting and engaging atmosphere for residents
• Support a respectful and positive work environment through excellent communication and interpersonal skills
• Demonstrate C.A.R.E. values (Compassion, Accountability, Relationships, and Excellence) with residents, family members, customers, and staff

Required Qualifications

• Current and valid Licensed Practical Nurse license in the state of Indiana
• Proficient medication management skills
• Ability to conduct thorough assessments and accurately document changes in resident condition
• Excellent communication and interpersonal skills
• Strong passion for geriatric nursing and commitment to senior care excellence
