Job Description

Clark Rehab and Skilled Nursing is seeking a Licensed Practical Nurse (LPN) to join our care team on a full-time days schedule. In this role, you will provide direct nursing care to residents in our skilled nursing facility, developing meaningful relationships while promoting their physical and psychosocial well-being. You will work collaboratively with nursing and interdisciplinary teams to deliver coordinated, comprehensive care that exceeds resident needs.

Responsibilities

  • Utilize nursing skills to promote the physical and psychosocial well-being of residents
  • Conduct thorough assessments and accurately document changes in resident condition
  • Manage medications with proficiency and accuracy
  • Create a comforting and engaging atmosphere for residents
  • Promote teamwork within the care team to meet resident needs
  • Support a respectful and positive work environment through excellent communication and interpersonal skills
  • Demonstrate C.A.R.E. values (Compassion, Accountability, Relationships, and Excellence) in all interactions with residents, families, customers, and staff

Required Qualifications

  • Current and valid Licensed Practical Nurse license in the state of Indiana
  • Proficient medication management skills
  • Ability to conduct thorough assessments and accurately document changes in resident condition
  • Strong passion for geriatric nursing and commitment to senior care excellence
  • Excellent communication and interpersonal skills

Schedule

  • Full-time days available

Compensation and Benefits

  • Salary: $29-$34 per hour based on years of experience, plus shift differential for nights and weekends
