Licensed Practical Nurse (LPN) Weekend Warrior

Countryside Manor is seeking Licensed Practical Nurses (LPNs) for a Weekend Warrior program in Fremont, Ohio. This position offers a consistent weekend schedule in a skilled nursing and long-term care setting. Both experienced LPNs and newly licensed nurses are encouraged to apply.

Clinical Setting

You will provide direct nursing care to residents in a skilled nursing facility and long-term care environment.

Responsibilities

• Provide direct nursing care to residents in a skilled nursing and long-term care setting
• Administer medications and treatments as ordered
• Monitor residents and assess changes in condition
• Document resident care accurately and timely
• Communicate changes in resident condition to physicians and appropriate members of the care team
• Follow individualized resident care plans
• Provide guidance and support to CNAs and other members of the nursing team
• Respond promptly to resident needs and concerns
• Maintain resident dignity, privacy, comfort, and safety
• Follow infection control, safety, and facility procedures
• Communicate effectively with residents, families, physicians, and healthcare team members
• Work collaboratively with the nursing team to provide excellent resident care

Required Qualifications

• Current Licensed Practical Nurse (LPN) license
• Ability to meet Weekend Warrior scheduling requirements
• Compassionate, dependable, and resident-focused approach
• Strong clinical, communication, and teamwork skills

Preferred Qualifications

• Long-term care, nursing home, or skilled nursing experience
