Licensed Practical Nurse, LPN

VitalCaring is seeking a Licensed Practical/Vocational Nurse (LPN/LVN) to provide direct patient care in home health settings within the West Palm Beach branch coverage area. In this field-based role, you will deliver skilled nursing services to patients in their homes, working under the supervision of a Registered Nurse while collaborating with an interdisciplinary care team.

As a Home Health LPN/LVN, you will be a key part of the care team, helping patients heal, recover, and maintain independence through consistent, hands-on care and meaningful patient relationships.

Responsibilities

• Provide direct patient care in accordance with the state Nurse Practice Act
• Implement and support the plan of care developed by the RN
• Deliver skilled nursing services, including treatments, medication support, and wound care
• Monitor patient condition and report changes to the RN and care team
• Educate patients and caregivers on self-care, treatments, and disease management
• Maintain infection control and proper use of equipment and supplies
• Document patient care accurately and timely within the EMR
• Communicate effectively with physicians, nurses, and interdisciplinary team members
• Participate in case conferences, team meetings, and ongoing training
• Support coordinated care by maintaining consistent communication across the care team

Required Qualifications

• Active LPN/LVN license in the state of practice
• Graduate of an accredited practical/vocational nursing program
• Current CPR certification
• Strong clinical and observational skills
• Clear communication and patient education abilities
• Ability to follow care plans while recognizing and reporting changes in condition
• Basic proficiency with EMR systems
• Valid driver's license and reliable transportation

Preferred Qualifications

• Minimum one year of nursing experience (Home Health, Med-Surg, or Acute Care preferred)
• Experience in home health or community-based care
• Strong patient engagement and relationship-building skills
• Ability to work independently while staying connected to a team
• Familiarity with Homecare Homebase (HCHB)
