Licensed Practical Nurse (LPN)

Vicdania Health Services is seeking a Licensed Practical Nurse for home care services in New Castle, Kent, and Sussex Counties. This position offers the opportunity to build lasting relationships with patients while providing individualized 1:1 care in a rapidly growing field.

Responsibilities

• Monitor patient conditions and assess their needs to provide the best possible care and education
• Observe and interpret patient symptoms and communicate findings to physicians
• Collaborate with physicians and nurses to develop individualized care plans for patients
• Perform routine procedures including blood pressure measurements and administering injections
• Administer medications and provide treatments according to physician orders
• Complete electronic charting using Axxess
• Foster a supportive and compassionate environment for patients and their families
• Expand knowledge and capabilities by completing assigned in-service training
