Licensed Practical Nurse LPN

Valor Healthcare is seeking a Licensed Practical Nurse to join our Community Based Outpatient Clinic (CBOC) in San Antonio. You will provide prescribed medical treatment and personal care services to patients with diseases and injuries, working collaboratively with the Primary Care Provider, RN, Medical Assistant, and expanded care team to develop and implement patient care plans.

Clinical Setting

This full-time position is based at one of two clinic locations in San Antonio, serving veterans and federal government service members in an outpatient primary care environment.

Responsibilities

Patient Care

• Assist with patient care, physical examinations, and ancillary tests
• Perform annual and routine nursing assessments and preventive health services authorized by the VA and clinic Medical Director
• Interview and collect patient information including pain assessment, vital signs (pulse rate, temperature, blood pressure), height and weight, and other relevant details
• Record information in the patient's electronic medical record
• Administer medications as ordered by a medical provider orally, subcutaneously, or intramuscularly; note time and amount in patient charts
• Apply hot and cold compresses as indicated and observe patients for adverse reactions, reporting to the clinic physician
• Operate electrocardiograph (EKG) and other diagnostic equipment including glucometers, pulse oximeters, nebulizers, oxygen equipment, spirometry, and eye exam equipment
• Dress wounds and incisions per physician instructions
• Collect lab specimens including urine, blood, and sputum for testing and perform waived laboratory tests
• Clean treatment rooms, equipment, and supplies using approved manufacturer wipes and solutions
• Submit laboratory and radiology results to medical providers and scan results into patient medical records after provider review
• Input patient information into VISTA/CPRS
• Complete clinical reminders due at each patient visit
• Assist with front desk duties including check-in, check-out, and phone coverage as needed
• Assist with co-managed care collaboration with non-VA providers and facilities
• Provide patient and family health education focused on self-management, prevention, wellness, and healthy lifestyle choices
• Advise patients on available community resources and services
• Maintain confidentiality of veterans' data and information
• Comply with all VA and company training requirements

Compliance and Clinical Outcomes

• Fulfill compliance requirements of the Office of Inspector General (OIG), Joint Commission (JC), Environment of Care (EOC) oversight, and lab compliance
• Remain focused on achieving excellent clinical outcomes through specified VA guidelines

Customer Service

• Provide excellent customer service to veterans both in person and over the phone, as well as to colleagues and clinic visitors
• Establish and maintain a warm, welcoming, and professional atmosphere
• Collaborate with clinic leadership to resolve veteran concerns or complaints

Enrollment Growth

• Participate in clinic outreach events to support enrollment initiatives
• Support patient enrollment and retention in appointments, follow-up visits, and nurse visits

Required Qualifications

• Diploma or certificate from an accredited program of practical or vocational nursing as approved by the state
• Minimum one year of experience as an LPN/LVN in a related primary care or ambulatory care setting
• Current, valid, unrestricted LPN/LVN license in the state of employment or any U.S. state or territory as required by VA contract
• Credentialed and in good standing through the Veterans Health Administration (VA)
• Current Basic Life Support (BLS) certification (valid for two years as specified by the certifying agency, such as AHA)
• Current ACLS certification as specified by individual VAMC contract
• Proficiency in written and spoken English
• Strong computer skills including electronic medical record (EMR) experience
• This is a safety-sensitive position

Final compensation will be based on relevant factors including work experience, skills, certifications, and internal equity.
