Licensed Practical Nurse (LPN)

Blue Hills Health and Rehab Center is seeking a Licensed Practical Nurse (LPN) to provide direct patient care to residents in our long-term care and skilled nursing facility located in Stoughton, Massachusetts. In this role, you will deliver comprehensive nursing care, administer medications, monitor vital signs, and assist residents with daily living activities while working collaboratively with an interdisciplinary healthcare team.

Responsibilities

• Deliver high-quality direct patient care to residents, including administering medications, performing vital signs, and assisting with daily living activities
• Conduct routine assessments and monitor resident progress to identify any changes or concerns
• Develop and implement individualized care plans in collaboration with the interdisciplinary team
• Provide emotional support and maintain a positive, respectful attitude towards residents
• Maintain accurate and up-to-date records of patient care and treatment plans
• Participate in quality improvement initiatives to continuously improve care delivery processes

Required Qualifications

• Current LPN license in the State of Massachusetts
• Experience in long-term care or skilled nursing setting
• Strong communication, interpersonal, and problem-solving skills
• Ability to work effectively in a fast-paced environment and prioritize tasks efficiently
