Licensed Practical Nurse (LPN) | Symbii Home Health & Hospice | Pocatello, ID

Symbii Home Health & Hospice is seeking a Licensed Practical Nurse (LPN) to provide direct patient care in the home health and hospice setting in Pocatello, Idaho. In this role, you will implement plans of care, provide patient and family education, coordinate with the interdisciplinary team, and maintain accurate documentation.

Responsibilities

• Provide direct patient care as defined in Idaho's Nurse Practice Act
• Implement the plan of care initiated by the registered nurse
• Provide accurate and timely documentation consistent with the plan of care
• Educate patients and families about their diagnosis and plan of care
• Coordinate home health services and report identified needs for other disciplines (HHA, OT, PT, MSW, ST, Dietician) to the registered nurse or clinical supervisor
• Use equipment and supplies effectively and efficiently
• Adhere to organizational documentation, care procedures, and standards of professional conduct
• Participate in personal and professional development
• Perform other duties as assigned by the registered nurse

Required Qualifications

• Current LPN license in the State of Idaho
• Current driver's license and auto insurance policy
• Reliable vehicle for travel to patient homes

Preferred Qualifications

• Home health experience
