Licensed Practical Nurse (LPN)

We are seeking a Licensed Practical Nurse (LPN) to provide direct nursing care and support daily clinical operations within our skilled nursing facility. This role ensures residents receive quality care in accordance with established standards and regulations.

Clinical Setting

This position is based in a skilled nursing facility serving residents who require ongoing medical support and daily assistance.

Responsibilities

• Administer medications and treatments as ordered
• Monitor, record, and report changes in resident condition
• Document assessments and care provided in the electronic medical record
• Assist residents with daily needs as appropriate
• Communicate effectively with residents, families, and interdisciplinary team members
• Maintain infection control, safety, and confidentiality standards
• Support the nursing team to ensure consistent, high-quality care delivery

Schedule

Full-time and part-time positions available. Shifts include 7a-7p and 7p-7a. Availability to work various shifts, including weekends and holidays, is required.

Required Qualifications

• Current LPN license in the state of employment
• Graduate of an accredited practical nursing program
• Strong communication and clinical skills
• Ability to work both independently and collaboratively
• Detail-oriented with effective time-management skills

Preferred Qualifications

• Electronic medical record (EMR) experience
