Licensed Practical Nurse - LPN

Skyline Nursing and Rehabilitation is seeking a Licensed Practical Nurse (LPN) to join our skilled nursing and rehabilitation team. In this role, you will provide direct patient care to seniors in a supportive environment, working with an interdisciplinary team to deliver high-quality nursing services.

Clinical Setting

This position is based at Skyline Nursing and Rehabilitation, a skilled nursing and rehabilitation facility serving seniors.

Responsibilities

• Assist in developing and implementing individualized treatment plans for residents
• Provide essential nursing services and direct patient care
• Conduct and document thorough medical evaluations upon admission and throughout treatment
• Supervise and support CNAs, including participation in their evaluations
• Oversee the Falls Program and Pressure Sore Program to ensure resident safety and well-being
• Perform additional duties assigned to support the best possible care for residents

Employment Type and Schedule

Full-time, part-time, and PRN positions are available. Shift options include mornings, afternoons, and nights, with additional hours available by request.

Required Qualifications

• Active Licensed Practical Nurse (LPN) license in good standing
