The opportunity

Job Description

REM Community Services, part of the Sevita family, is seeking a Licensed Practical Nurse to provide medical treatment and personal care services to individuals with intellectual and developmental disabilities in a community-based setting. You will work under the direction of an RN or physician to deliver high-quality, mission-driven care.

Responsibilities

  • Provide basic care and medical treatment to individuals receiving services
  • Complete health assessments and provide input on health diagnoses to the RN or physician for development of nursing care plans
  • Follow up on all physician orders and monitor individuals' medical and dental appointments
  • Monitor and record medications in a timely manner; act as a liaison to pharmacy
  • Review medication error forms, analyze errors, and make recommendations for procedural changes
  • Administer medications as required
  • Oversee and monitor staff who administer medication
  • Conduct quality assurance checks on health care records
  • Provide emergency coverage and care for individuals receiving services
  • Maintain confidentiality, respect human rights, and practice universal precautions in accordance with organizational policy
  • Work with external agencies to coordinate services and ensure consistent treatment of clients
  • Administer, track, and maintain logs for staff and individual testing
  • Provide training to staff on medication administration, universal precautions, and infection control
  • Provide training to individuals on self-medication, prescribed treatments, sex education and STD prevention, and smoking cessation
  • Coordinate, organize, and assist with household activities such as light housekeeping and meal preparation
  • Transport individuals to planned and necessary activities and appointments
  • Participate in on-call system as needed
  • Perform other related duties as required

Schedule

  • Day shift, various days Monday through Friday, 8:00 AM to 4:00 PM
  • On-call for weekends and evenings as needed
  • Full-time position

Compensation

  • $23.00 per hour

Required Qualifications

  • Current LPN/LVN licensure for state of residence
  • Associate degree in nursing or related field, or state certificate
  • One year of experience working with the population served in a nursing capacity
  • Valid driver's license held for at least one year (post-permit)
  • Clean motor vehicle record meeting Sevita's insurability policy
  • CPR/First Aid certification
  • HIV/AIDS certification
  • Bloodborne Pathogen (BBP) certification
  • ACT training

Physical Requirements

  • Heavy work: exerting up to 100 pounds of force occasionally, up to 50 pounds of force frequently, and up to 20 pounds of force constantly to move objects
  • Potential exposure to blood, body fluids, or tissues as part of job duties

Additional Requirements

  • Travel as needed
  • Ability to perform essential job functions either unaided or with reasonable accommodation as determined on a case-by-case basis
