Licensed Practical Nurse (LPN)

Sarah Neuman Center for Rehabilitation and Nursing is a 301-bed skilled nursing facility located in Mamaroneck, New York, seeking a Licensed Practical Nurse (LPN) for weekend positions. You will deliver nursing care to patients and residents requiring long-term or rehabilitative care, working under direct supervision in accordance with state-specific Nurse Practice Act regulations, facility policies and procedures, and nursing judgment.

Responsibilities

• Manage admissions, discharges, and transfers of residents
• Administer medications and coordinate medication requisitions with pharmacy and physicians
• Collect patient and resident data, make observations, and report pertinent information related to care
• Implement the patient and resident plan of care and evaluate patient and resident response according to state-specific regulations
• Direct and supervise care given by other nursing personnel in selected situations, in accordance with state-specific regulations
• Identify and report all abnormal changes in residents' condition to proper personnel per center policy
• Follow infection control, emergency, and other center policies
• Collaborate with the resident's interdisciplinary team to provide the best outcomes
• Maintain knowledge of necessary documentation requirements
• Maintain knowledge of equipment setup, maintenance, and use, including monitors, infusion devices, and drain devices
• Maintain confidentiality and patient and resident rights regarding all patient, resident, and personnel information

Required Qualifications

• Active LPN (Licensed Practical Nurse) license issued by the State
• CPR Certification

Preferred Qualifications

• Two years of prior nursing experience
