Licensed Practical Nurse (LPN)

Samaritan Daytop Village, a nationally recognized comprehensive Health and Human Services Agency serving over 33,000 New Yorkers annually, is seeking a Licensed Practical Nurse (LPN) to support providers in delivering patient care across our 60+ programs throughout New York City and the greater New York area.

Under the general direction of the RN and/or Physician, you will assess, plan, evaluate, and deliver nursing care in collaboration with the clinical team. You will work with clients with Substance Use Disorder (SUD) and/or Serious Mental Illness (SMI) in support of the agency's mission and goals.

Responsibilities

• Assist with patient care including checking vital signs, blood pressure, pulse rate, breathing, temperature, and weight
• Discuss chief health concerns with patients
• Administer injections and perform tests
• Take patient medical history and pretreatment information, entering data into the computer system
• Follow orders of the physician and registered nurse in providing care
• Record detailed reports of care given in patients' medical records
• Maintain proper charting, documentation, and medical record maintenance
• Check, clean, and dress wounds
• Reinforce instructions given to patients by the RN or Physician
• Notify the RN or Physician of significant findings or changes in patient's physical or mental condition
• Place phone calls to pharmacies to order prescriptions
• Maintain confidentiality of patient records
• Pull and prepare patient charts
• Stock patient rooms with supplies
• Maintain neatness of work area
• Provide emotional support as needed
• Initiate emergency assistance according to policies and procedures
• Attend staff development trainings and nursing conferences
• Keep skills current and increase knowledge and productivity
• Perform other duties as requested

Required Qualifications

• Active New York State LPN license
• BLS certification
• High school diploma or GED
• Graduate of a formal Technical or Vocational LPN Program
• At least one year of experience as an LPN working with clients with Substance Use Disorder (SUD) and/or Serious Mental Illness (SMI)
• Available to work a flexible schedule as needed in response to participant and staff needs
• Computer literacy including proficiency in Microsoft Office Suite and EHR systems
• Ability to put patients at ease and explain physician's instructions
• Ability to multi-task with excellent organization skills
• Experience working directly with people from diverse racial, ethnic, and socioeconomic backgrounds
• Knowledge of medical terminology
• Ability to work as a team player and independently

Compensation

• $37.36 to $39.06 per hour
