Impact Life is seeking a part-time Licensed Practical Nurse (LPN) to support clinical and care coordination services in rural Sussex County. Working under the supervision of a Nurse Practitioner, you will provide nursing care to participants with complex medical, behavioral health, and social needs, coordinating across healthcare, behavioral health, substance use treatment, recovery support, and community services.
Responsibilities
- Provide nursing services within the LPN scope of practice under appropriate clinical supervision
- Obtain and document vital signs, health histories, medication information, and other clinical data
- Support the Nurse Practitioner in coordinating participant healthcare needs
- Provide health and medication education within scope of practice
- Support medication reconciliation, adherence, and follow-up as directed by the prescribing provider
- Assist participants with medical appointments, laboratory testing, referrals, and other healthcare needs
- Identify barriers to accessing or remaining engaged in healthcare and communicate those needs to the team
- Coordinate with behavioral health, treatment, peer recovery, and community-based staff
- Support transitions between healthcare, treatment, recovery, and community settings
- Follow up with participants following medical visits, referrals, hospitalizations, or other transitions as directed
- Communicate changes in health status or clinical concerns to the appropriate provider
- Maintain timely and accurate documentation
- Participate in interdisciplinary meetings, care coordination, quality improvement, and program activities
Required Qualifications
- Current Delaware LPN license or eligibility for Delaware licensure
- Graduate of an accredited practical nursing program
- CPR/BLS certification or ability to obtain as required
- Strong communication, organization, and documentation skills
- Ability to work effectively as part of a multidisciplinary team
Preferred Qualifications
- Experience in behavioral health, substance use disorder treatment, primary care, community health, or another community-based healthcare setting
- Experience working with individuals with complex medical, behavioral health, or social needs
- Knowledge of substance use disorder, MOUD, harm reduction, and recovery-oriented care
