Licensed Practical Nurse (LPN)

Rock River Nursing & Rehab in Fort Atkinson, Wisconsin is seeking a Licensed Practical Nurse (LPN) to provide direct patient care in our long-term care and rehabilitation setting. In this role, you will work with residents of varying acuity levels, building meaningful relationships while delivering compassionate, high-quality care.

Responsibilities

• Provide direct patient care, including administering medications, wound care, and physical therapy interventions
• Collaborate with the interdisciplinary team to develop and implement individualized care plans that meet each resident's unique needs
• Assist with admissions, transfers, and discharges, ensuring smooth and efficient transitions
• Maintain accurate and detailed records of patient care, treatments, and medications administered
• Participate in quality improvement initiatives and contribute to maintaining high levels of patient satisfaction

Clinical Setting

You will work in a nursing and rehabilitation facility providing care to residents requiring long-term care and rehabilitation services.

Required Qualifications

• Current LPN licensure in the state of Wisconsin
• High school diploma or equivalent
• Minimum one year of experience as an LPN

Preferred Qualifications

• Associate's degree or higher in nursing or related field
• Experience in long-term care and/or rehabilitation settings

Schedule

This is a per diem position offering flexible, as-needed work. You will be expected to work a variety of shifts, including evenings, weekends, and holidays.
